Description

These programs will create an interdisciplinary team of an OMH Supportive Housing case manager, a local office for the aging case manager, a homecare professional, and at least one paid peer professional who will collaborate with the OMH supportive housing residents to identify outstanding service and social needs, building a person-centered, holistic plan that is based on the older adult’s priorities and preferences for aging in place. The OMH supportive housing provider, as the lead applicant, will also partner with local community organizations to meet social determinants of health and social engagement needs. The OATAC will be responsible for training and technical assistance focused on programmatic and fiscal strategies to support the planning, implementation, operation, and evaluation of the service demonstration programs. Please see RFP for full details and funding info: https://omh.ny.gov/omhweb/rfp/ All proposals to this RFP must be submitted through the SFS. Budget estimate

$2,500,000 – $3,000,000

The solicitation does not disclose a total award amount. Based on a 5‑year statewide grant for technical assistance and training in a high‑need health‑social service area, similar New York grants have ranged from $500 K to $3 M total, depending on the scope of services, number of training events, and staffing levels. A conservative estimate would be $500 K per year ($2.5 M total) for a modest operation, while a more ambitious implementation could approach $600 K per year ($3 M total). Factors influencing the final budget include the number of participating housing sites, the depth of fiscal consulting required, and any additional evaluation or reporting deliverables.
