The Ohio Department of Health Office of Human Resources 58032 SFY26_27 Respirator Fit Testing

The Ohio Department of Health is seeking a provider to perform respirator fit testing for its employees. The provider must have trained individuals who can perform fit testing and make the determination of medical clearance and respiratory fit testing. The provider must also have occupational health locations that are accessible for ODH employee respirator fit testing throughout various locations in Ohio. The scope of work includes completing respirator clearance, physical exams, and fit tests, and providing documentation to employees. • The contract term is 24 months with two possible 24-month renewals. • The project budget is not to exceed $49,000.00 per year. • The provider must have the ability to perform the three components of fit testing: review of a medical clearance form, employee physical if results of clearance form warrant exam, and respiratory fit test. • The provider must submit a technical proposal and cost proposal as part of its total proposal. • The proposal will be evaluated based on the provider's documentation of trained individuals, occupational health locations, and ability to perform the three components of fit testing, as well as its work plan and cost proposal.
