G004 Homeless Care for Homeless Veterans, Akron Ohio

The Department of Veterans Affairs is seeking a contractor to provide Homeless Care for Homeless Veterans (HCHV) services in Akron, Ohio. The contractor will be required to deliver services using a "Safe Haven" model of residential care, providing a safe and secure environment for up to 12 homeless veterans. The services will include room and board, case management, therapeutic and rehabilitative services, and assistance with permanent housing search. The contractor will be expected to maintain a minimum staffing requirement ratio of one full-time dedicated case manager per every 12 active veterans and provide services that meet the unique needs of homeless women veterans. The contract will have a performance period of up to 6 months, with the possibility of extensions. Some key features of the service include: • Providing a safe environment for all veterans consistent with the Safe Haven model of housing and services • Offering a low-demand environment with a minimum set of rules designed to re-establish trust in the chronically homeless veteran • Providing therapeutic and rehabilitative services, including group activities, case management, and counseling • Assisting veterans with permanent housing search and providing direct and ongoing assistance in achieving permanent housing • Maintaining daily sign-in sheets signed by veterans and submitting them to the HCHV Liaison monthly, along with invoices
