Description
The Department of Veterans Affairs is seeking information and sources that can provide Prostate Testing and Reporting for the Salt Lake City VA Medical Center. The contractor will provide prostate testing and reporting, which includes laboratory testing, analysis, and result interpretation. The testing report will provide personalized treatment and identify patients at high versus low risk of progression of cancer. The contract will be awarded as a firm fixed price contract as a blanket purchase agreement with a 5-year ordering period. Some key requirements include:
* Providing individual patient-chartable test result reports meeting Clinical Laboratory Improvement Amendments (CLIA) requirements
* Maintaining safety and health standards consistent with the requirements set forth by the Occupational, Health and Safety Administration (OSHA)
* Providing encrypted and/or secured electronic transmission of all completed and/or partial test results to the ordering activity within published turnaround time of fifteen days
* Complying with the Health Insurance Portability and Accountability Act (HIPAA)
* Providing and maintaining quality assurance programs which meet federal regulations for laboratory operations
