Chronic Disease Management Programming

The State of South Dakota’s Department of Health is seeking health‑care entities to design, implement, and manage evidence‑based chronic‑disease‑management interventions and a remote‑patient‑monitoring (RPM) program for rural and frontier communities. The contractor will deliver both clinical and community‑based services, purchase and deploy RPM equipment, and provide monthly progress and financial reports. The contract runs from July 1 2026 through October 30 2027 with up‑to‑three one‑year renewal options. • Implement two‑component program: (1) evidence‑based chronic‑disease interventions (e.g., care coordination, screening, education) and (2) RPM equipment deployment linked to those interventions. • Provide monthly performance reports and itemized expenditure claims; comply with insurance, indemnity, and data‑ownership requirements. • Budget must include personnel, supplies, travel, “other” costs (including RPM hardware), and indirect costs (max 6.3 %). Preference is given to organizations located in or serving rural South Dakota.
