On-Site Health and Wellness Center

The City of Rock Hill is seeking proposals from qualified companies to provide On-Site Health and Wellness Services to City employees, early retirees, and their dependents. The services include primary health care, acute and episodic health care, wellness services, health screenings, and identification and outreach to patients exhibiting markers for certain chronic health conditions. The City requires the proposer to demonstrate robust data and analytics capabilities, provide a detailed staffing model, and describe how they will manage health records and ensure patient confidentiality. The proposer must also provide a forward-looking strategy for program development and describe how they will measure and report outcomes. Some key services that the proposer must provide include: • Wellness care focused on prevention and wellness with a proactive evidence-based approach • Adult and child primary care services • Episodic/acute care for illness and injury for patients aged six months and older • Health risk assessments • Immunizations and injections • New hire physicals • Patient exams and screenings • Annual adult physicals • Gynecology physicals • Written prescriptions • Occupational-related screenings, including but not limited to SC Department of Transportation Commercial Driver License (CDL) physicals • Occupational-related physicals for firefighters • Occupational-related pre-employment physicals for police officers
