Description

The Maryland Department of Health is seeking information from providers to establish a robust, person-centered Care Coordination Team for the Assisted Outpatient Treatment (AOT) Program. The program aims to reduce hospitalization and incarceration rates, improve treatment engagement, and enhance the quality of life for individuals with serious and persistent mental illness. The Care Coordination Team will consist of a Psychiatrist, Case Manager, and Certified Peer Recovery Specialist, and will provide comprehensive, coordinated care to participants. • The program will serve individuals with severe and persistent mental illness who face significant barriers to voluntary treatment adherence. • The Care Coordination Team will work collaboratively to formulate and monitor an AOT Treatment Plan. • The team will provide evidence-based and recovery-oriented care, including psychiatric assessment and medication management, crisis intervention and de-escalation skills, and co-occurring substance use disorder management. • The program will also provide community engagement, public education, and family and caregiver support. • Providers are asked to submit information on their company, services, and experience in providing care coordination and treatment services for AOT programs.
