Chinle Hospital preventive maintenance agreement (PMA) for Radiology Shimadzu RadSpeed Radiographic X-Ray system equipment

The Indian Health Service (IHS) is seeking a contractor to provide preventive maintenance agreement (PMA) services for Radiology Shimadzu RadSpeed Radiographic X-Ray system equipment at the Chinle Comprehensive Health Care Facility. The contractor will be responsible for performing routine maintenance, repairs, and replacements of parts and components to ensure the equipment is in good working condition. The contract will be for a period of one year, with options to extend for an additional year. The contractor must have experience in providing PMA services for similar equipment and must be able to provide documentation of their experience and qualifications. • The contractor must provide a detailed pricing proposal, including a breakdown of costs for labor, materials, and travel. • The contractor must provide a description of their experience and qualifications in providing PMA services for similar equipment. • The contractor must provide a list of references, including contact information for at least two previous clients. • The contractor must be able to provide proof of insurance and liability coverage. • The contractor must be able to provide a certificate of compliance with all applicable laws and regulations.
