Description
The Maryland Department of Health is seeking a CDC-approved vendor to provide lifestyle coach training and advanced training opportunities for the National Diabetes Prevention Program in Maryland. The vendor will provide three virtual lifestyle coach training events and at least 200 hours of Advanced Coach Training opportunities for current lifestyle coaches and program coordinators. The training events should have a minimum of 12 and a maximum of 15 participants, and the vendor will be responsible for setting up and maintaining the virtual platform. The vendor should also provide reporting and technical assistance to the Department and lifestyle coaches. Key requirements include:
• Providing confirmation of lifestyle coaches successfully completing training
• Providing evaluation results for each lifestyle coach training
• Providing advice to the Department and lifestyle coaches regarding DPP implementation
• Providing ongoing technical assistance to coaches who have completed the Lifestyle Coach Training
The contract will be awarded to the responsible and responsive bidder with the most favorable bid price (lowest cost).
