Description
This statewide fixed‑price contract provides Family Resource Connection and Preservation Services to families referred by the South Carolina Department of Social Services. The contractor will deliver weekly case‑rate services, conduct assessments using the FAST tool, provide motivational‑interviewing‑based counseling, and supply up to $500 per family in concrete supports (housing assistance, transportation, etc.). Service delivery must be available 24/7, with a minimum of one visit per week and up to four visits per week for high‑need families. The contract runs for five years (Oct 6 2024 – Oct 6 2029) with four optional one‑year extensions, and the State will pay a weekly rate of $362.83 per family plus reimbursable concrete services.
