HealthySteps Implementation Pilot

The County of Riverside is seeking proposals from Federally Qualified Health Centers and/or pediatric and family medicine primary care practices to implement the evidence-based primary care prevention model, HealthySteps. The contract is for a maximum investment of $3.9 million for a 3-year pilot project.
