Check Stock Paper

The Commonwealth of Kentucky is soliciting bids for the provision of check stock paper. The solicitation includes four line items: Check Forms 28# Safety Paper Z-Fold, Check Forms 28# Safety Paper C-Fold, W-2 Form/Z-Fold Mailer, and Z-Fold Mailer. The successful bidder will be required to provide the specified quantities of each item. The bid submission deadline is 12/05/2025 at 13:30. • The scope of work includes providing check stock paper that meets the specified requirements. • The successful bidder will be required to comply with the terms and conditions set out in Attachment A. • The bid must be submitted electronically or with a signature of an authorized agent. • The vendor must read and understand the solicitation and the terms and conditions before submitting a bid.
