Description
The Henry County Board of Commissioners is seeking bids from qualified vendors to provide medical supplies and equipment required by the County's Fire Department as an annual contract. The contractor shall furnish all materials, labor, tools, equipment, and services necessary in the performance of this bid. The contract will be for a period of one year, with the option to renew. The bid includes various medical supplies and equipment, such as medical supplies and pharmaceuticals, repair kits, hospital and medical equipment, emergency medical services equipment and supplies, gloves and finger cots, medical ergonomic instruments and equipment, and medical examination equipment and supplies. The bidder must provide a bid price sheet, which includes the percentage off medical supplies and equipment, and any additional discounts offered by the company. The bidder must also provide references, a non-conflict of interest statement, and a Georgia security and immigration compliance act affidavit and agreement.
• The bid must be submitted in a sealed envelope or package, with the label attached to the outside, by the due date and time.
• The bid must include all required documents, including the solicitation form, addendum cover sheet, bid authorization affidavit, non-conflict of interest statement, references, Georgia security and immigration compliance act affidavit and agreement, covenant of non-discrimination form, supplier inclusion program, bid price sheet, and checklist for bid documents.
• The bidder must provide a W-9 form and any other required forms.
• The bidder must acknowledge any addenda and provide a signed addendum acknowledgement form.
