Family Professional Partnership

The Connecticut Department of Public Health is seeking proposals to develop, implement, evaluate, and administer regionally based medical home care coordination services through a Family Professional Partnership Outreach and Education Program. The program aims to support Children and Youth with Special Health Care Needs (CYSHCN) and providers of services for CYSHCN. Services will include family/caregiver support services, outreach and education for families, access to primary care and community providers, and assistance for families with referrals to community resources. The contract will be for three years, plus one, plus one, beginning July 1, 2026, through June 30, 2031, subject to the availability of funds and satisfactory program performance.
