Description
The County of Shelby is seeking a qualified public or nonprofit organization to provide District Long‑Term Care Ombudsman Services for the Aging Commission of the Mid‑South. The contractor will serve the four‑county district (Fayette, Lauderdale, Shelby, and Tipton), maintain a full‑time Ombudsman, manage volunteers, conduct regular facility visits, handle complaints, and submit required monthly and quarterly reports to the State Ombudsman. The award will be a fixed‑term contract with an initial period ending June 30 2027 and up to two optional one‑year extensions, subject to performance and fund availability.
• Must be a public or nonprofit entity with ≥ 5 years experience delivering long‑term‑care ombudsman services.
• Required staffing: at least one full‑time District Ombudsman plus supervisory structure and volunteer coordination.
• Mandatory compliance: Tennessee licensing, Shelby County Business Registration Number (BRN), EOC registration, Title VI, drug‑free workplace affidavit, conflict‑of‑interest disclosures, and appropriate insurance.
• Deliverables include monthly/quarterly reports, an up‑to‑date facility database, a customer‑satisfaction survey (≥ 80 % positive), and adherence to a quality‑assurance plan.
• Proposal must be submitted in hard copy (original + 4 copies) by April 10 2026, 4:00 PM CST, with a completed Excel scorecard and required certifications.
