Description
The Colorado Department of Health Care Policy and Financing is soliciting proposals from experienced organizations to provide Housing Navigation Services for Region 5B, which includes Moffat, Routt, Grand, Rio Blanco, and Summit counties. The selected contractor will be responsible for coordinating rental assistance vouchers, administering housing navigation services, and providing community outreach. The contractor will work with Medicaid members, transition coordinators, and other stakeholders to identify and secure affordable and accessible housing options.
• Key responsibilities include:
• Coordinating rental assistance vouchers and submitting applications to the Department of Housing
• Building relationships with the housing community to increase awareness of limited housing options
• Providing information to community entities about accessible and affordable housing needs
• Networking with landlords, housing agencies, and community organizations to promote identification and development of affordable and accessible housing
• Submitting monthly reports on housing navigation services, community outreach, and other deliverables
