Guardianship Services Master Agreement Request for Response

The Commonwealth of Massachusetts Department of Mental Health is establishing a statewide pool of qualified individuals and nonprofit corporations to provide court‑appointed guardianship and/or conservatorship services for persons receiving DMH services. Selected contractors will be paid on a rate‑based “rate contract” structure, performing quarterly meetings, report filings, and other fiduciary duties as required by probate courts and DMH. The solicitation is open‑ended, with an initial 10‑year term (plus a 3‑month extension for first‑period awards) and a procurement end date of June 30 2033, allowing new respondents to submit until that date. • Services include quarterly caretaker meetings, Individual Service Plan participation, emergency availability, court‑required reporting (initial 60‑day and annual reports), and, where applicable, financial inventory and account management for conservatorship. • Compensation is set at $60.00 per hour for guardianship work, $30.00 per hour for travel time, with mileage and ancillary costs reimbursed up to stated caps; no maximum dollar ceiling is imposed. • Qualification requirements: non‑profit corporate status or individual professional standing, sound financial condition, ability to provide required bonds/insurance, CORI background check, and completion of numerous mandatory forms (Standard Contract, W‑9, Authorized Signatory Listing, EFT Authorization).
