Description
The Massachusetts Department of Mental Health (DMH) is establishing a Master Agreement (MA) to create a pool of qualified General Reference Laboratory Service Providers (GRLSP) that can deliver a full range of clinical reference laboratory testing and associated phlebotomy services across the state. Qualified vendors will be evaluated on licensing, accreditation, financial stability, liability insurance, and ability to meet 24 × 7 service expectations, including STAT specimen pick‑up, result reporting, and on‑site education. The MA has a five‑year base term with up to five one‑year renewal options, allowing contracts to run as long as ten years. Pricing for each test is submitted via a standardized price‑sheet (Attachment D) and is fixed for the first 12 months, after which annual adjustments are permitted. Vendors must submit extensive certifications, liability coverage proof, and a Supplier Diversity Plan if anticipated spend exceeds $150 K.
