Description
The Colorado Department of Health Care Policy and Financing is soliciting proposals for Residential Navigation Services in Region 5B, which includes Moffat, Routt, Grand, Rio Blanco, and Summit counties. The selected contractor will provide housing navigation services to individuals with disabilities and the elderly, including coordination of rental assistance vouchers, administration of housing navigation services, and community outreach. The contractor will be responsible for building relationships with the housing community, providing information to community entities, and networking with landlords, housing agencies, and community organizations to identify affordable and accessible housing options. The contract is expected to begin on January 1, 2026, and end on December 31, 2026, with a total duration of one year. The Department may exercise options to extend the contract beyond the initial term.
