RFI for Program of All Inclusive Care for the Elderly (PACE) Expansion

The Louisiana Department of Health (LDH) is seeking information from organizations interested in developing a Program of All-Inclusive Care for the Elderly (PACE) in the Shreveport area. The goal of this Request for Information (RFI) is to gather information on the development of a PACE program, including the specific areas to be served, potential barriers, and resources needed to support the program. PACE is a community-based care model that enables individuals to live in the community for as long as possible, while receiving comprehensive medical, physical, emotional, and social services. The ideal respondent will have experience serving older adults with disabilities and/or chronic conditions, as well as individuals with serious mental illness. Key requirements include: • Experience operating a PACE facility • Ability to develop a provider network • Financial capacity to fund program development and start-up costs • Ability to manage comprehensive care for a complex nursing facility eligible population • Ability to accept and serve nursing home residents who are eligible for PACE and wish to move into the community The RFI response should include information on the organization's mission, history, and experience, as well as a financial plan, marketing plan, and discussion of needed resources to support the program.
