Description
The Fermi Forward Discovery Group, LLC is seeking a subcontractor to perform the MIPV 01-02 Groundbed and Rectifier Replacement project. The project requires the replacement of the existing groundbed and rectifier at the Fermilab facility. The subcontractor will be responsible for all work described in the Statement of Work, including transportation, overhead, bonding costs, insurance, safety oversight, quality control oversight, and supervision. The project is expected to be completed within 10 days.
• The subcontractor must have at least five years of electrical experience in industrial facilities and at least five years of excavating/paving experience.
• The subcontractor must provide descriptions of its experience performing projects of similar size, scope, and complexity.
• The subcontractor must submit resumes for the project manager and project superintendent, demonstrating relevant experience and training.
• The subcontractor must provide safety documents, including the Fermilab Subcontractor Injury & Illness Data Questionnaire and the National Council on Compensation Insurance (NCCI) Workers Compensation Experience Rating form.
• The subcontractor must provide a corporate quality assurance plan.
