Request for Qualifications For Healthcare Ancillary Services Master Agreement

The Los Angeles County Department of Health Services is seeking qualified vendors to enter into a Master Agreement to provide various Healthcare Ancillary Services. The selected vendors will be required to meet the Minimum HASMA Qualifications and the Minimum Mandatory Requirements specified in the applicable Work Order Solicitation. The Master Agreement will have a term of approximately 3 years, with the option to extend for up to 3 additional years. The scope of work includes providing Healthcare Ancillary Services, and the vendors will be reimbursed based on the terms of the Master Agreement and the applicable Work Order. • The Minimum HASMA Qualifications include business status verification, submission of required forms, and vendor non-responsibility review. • The Minimum Mandatory Requirements will be specified in the applicable Work Order Solicitation. • The Master Agreement will be based on the standard required provisions set forth in Appendix A – Master Agreement. • The County reserves the right to waive any immaterial deviations in any submitted Response and to suspend or cancel the RFQ at any time.
