Description
The Agency of Human Services - Office of Health Care Reform is seeking to establish contracts with one or more companies to provide technical assistance support to hospitals for health care transformation. The selected vendor will support hospitals in implementing care transformation initiatives, provide data-informed implementation and reporting, and develop evaluation and sustainability plans. The vendor should have experience working with hospitals, particularly in Vermont, and have expertise in value-based payments, population health management, and financial planning.
• Provide technical assistance support to hospitals for health care transformation
• Develop implementation plans with clear timelines and measurable milestones
• Develop evaluation and sustainability plans and report on metric performance
• Provide standardized tools, templates, dashboards, and reports to track progress
• Offer technical assistance around payer-specific financial planning and reimbursement models
