81622P0004-Justice-Involved Supportive Housing (Open-Ended)

The Department of Health and Mental Hygiene (DOHMH) is seeking proposals for the provision of permanent supportive housing units for adults with mental illness and/or substance use disorder who are frequently cycling through jail and shelter. The goal of the Justice-Involved Supportive Housing (JISH) program is to provide rapid access to supportive housing units, case management services, and other resources to prevent further use of jail, shelter, or other emergency services. The program aims to decrease homelessness and criminogenic behaviors. Proposers may propose for one or both of the two housing types: Scattered Site and Congregate. The contract term is anticipated to be 9 years, contingent on the availability of funding. Some key aspects of the program include: • Providing case management services to link tenants to health and behavioral health services • Maintaining housing, linking and engaging with service providers, and supporting positive integration into the community • Tracking and reporting information on tenant demographics, income source, place discharged to, and outcome data • Establishing baseline data to track linkage to primary and behavioral health care providers, hospitalization rates, shelter stays, incarceration, and employment • Meeting key program indicator goals, such as linking tenants to a primary care physician, maintaining apartments stably, and reducing contacts with the criminal justice system.
