Description
The Colorado Department of Health Care Policy and Financing is seeking a contractor to provide Housing Navigation Services for Region 5B, which includes Moffat, Routt, Grand, Rio Blanco, and Summit counties. The contractor will be responsible for coordinating rental assistance vouchers, administering housing navigation services, and providing community outreach. The contractor will also be required to submit monthly reports and participate in evaluation meetings. The goal of the project is to ensure improved housing search and placement for individuals receiving Targeted Case Management-Transition Coordination. Some key responsibilities include:
• Coordinating rental assistance vouchers and submitting them to the Department of Housing
• Building relationships with the housing community to increase awareness of limited housing options
• Providing information to community entities about accessible and affordable housing needs
• Networking with landlords, housing agencies, and community organizations to promote identification and development of affordable and accessible housing
• Submitting monthly reports on housing navigation services, including the number of members served, successful placements, and no-fault failed transitions
