Description
The Colorado Department of Health Care Policy and Financing is seeking a Local Contact Agency to provide In-Reach Counseling services to Medicaid members residing in skilled nursing facilities. The selected agency will be responsible for conducting Group In-Reach sessions, providing information about community-based services, and referring members to appropriate agencies. The agency will also be required to develop and submit a Group In-Reach Plan, provide quarterly meeting minutes, and update a Regional Resource Document. The contract is anticipated to last until June 30, 2027. Some key points to consider are:
• The agency must have experience working with Medicaid members and community organizations that serve the disability and elderly community.
• The agency must have knowledge of local and non-profit resources that could support adults with disabilities in the community.
• The agency must be able to provide sufficient personnel to perform the work, including a Project Lead with working knowledge of Medicaid benefits and community partners.
• The agency must be able to ensure that all work performed complies with applicable provisions of §§ 24-85-101, C.R.S., et seq., and the Accessibility Standards for Individuals with a Disability.
Some key requirements to consider are:
• The agency must conduct at least one Group In-Reach session at each Medicaid-certified nursing facility in the region per each 6-month period.
• The agency must use Department-approved materials and document attendance.
• The agency must ensure that attendees include Medicaid Members, Medicare Members, PACE participants, privately paying residents, legal guardians, POAs, and natural supports.
• The agency must provide Group In-Reach only to individuals age 18 and over.
• The agency must work with the Nursing Facility to disseminate an invitation to the Nursing Facility residents and their natural supports.
