The Illinois Department of Public Health (IDPH) is seeking applications for the Dementia Caregiver Program for Underserved Populations. The purpose of this grant program is to increase education, support, and community connection for caregivers of people living with Alzheimer’s Disease and Related Disorders (ADRD) from four diverse underserved groups: African American Caregivers, Hispanic/Latinx American Caregivers, Asian American Caregivers, and Caregivers in Rural Areas. The program aims to reduce stress, empower, and support caregivers from diverse populations, as well as reduce barriers and stigma that prevent access to care.
Key activities and deliverables include:
* Developing and delivering high-quality, culturally tailored dementia caregiver education
* Providing caregiver support via monthly accessible support groups
* Facilitating community connection via culturally appropriate in-person memory care cafes
* Tracking and evaluating performance measures and outcome data
Applicants must submit a project plan that covers the period and how the award will be executed. The project plan should include necessary details to enable the agency to manage the grant agreement activity against planned project performance.
