The Agency for Health Care Administration (AHCA) is issuing this Request for Applications (RFA) to establish grant agreements with eligible subrecipients under Florida’s Rural Health Transformation Program (RHTP). The RHTP is a five-year, federally funded initiative administered by AHCA under the Centers for Medicare and Medicaid Services (CMS) Cooperative Agreement. The Specialty and Acute Care Bundle is designed to strengthen rural health care facilities' capacity to deliver specialty, acute, and behavioral health care through shared technology platforms, integrated data infrastructure, and expanded telehealth service lines. The bundle consists of two required Core service components: Florida HIE/ENS Onboarding and Diagnostics Technology Support, and four elective telehealth lead initiatives: Behavioral Health Telehealth & Telehub Psychiatry, Tele-Imaging / Virtual Specialty Clinics, Tele-Intensive Care Unit / eICU, and Hub-and-Spoke Telestroke. Applicants must propose at least one elective telehealth initiative and may propose up to all four.
• The bundle aims to improve access to specialty and acute care services in rural areas.
• Applicants must demonstrate organizational capacity to deliver both Core service components and any additional telehealth initiatives proposed.
• The RFA includes a detailed scoring system to evaluate applications based on factors such as organizational capacity, lead telehealth initiative narrative, and sustainability plan.
