HEALTH HOME NON-MEDICAID CARE MANAGEMENT (2620)

Tompkins County is seeking a qualified agency to provide non-Medicaid care management services for high-need individuals with Serious Mental Illness (SMI) in Tompkins County. The selected provider will be responsible for providing care management services, including developing a plan of care, coordinating services, and providing support to individuals. The contract will be for one year, with the option for annual renewals based on performance. The provider will be required to submit quarterly and annual program and financial performance data to the county. • The provider must be a voluntary agency that operates an established NYS DOH Care Management Agency (CMA). • The provider must currently be providing care management services in Tompkins County, New York, and be in good standing. • The provider must meet the standards for designation by OMH as a Specialty Mental Health Care Management Agency. • The provider must have an established working relationship with Tompkins County Adult SPOA. • The provider must be able to demonstrate experience in providing care management services to SMI adults.
