DHS / Consumer Surveys

The Minnesota Department of Human Services is seeking a contractor to conduct consumer surveys in Minnesota nursing homes and assisted living settings, as well as National Core Indicator surveys for older adults and people with disabilities. The contractor will be responsible for delivering comprehensive survey services to support three large-scale initiatives. The surveys will include quality of life and experience surveys for long-stay and short-stay nursing home residents, family satisfaction surveys, and surveys of individuals receiving home and community-based services. The contractor will be required to use existing survey instruments and protocols, and to implement outreach strategies to reach key resident representatives. • The contractor will conduct in-person interviews with long-stay nursing home residents to collect quality of life data. • The contractor will conduct experience surveys of short-stay nursing home residents by mail, with an online option for respondents. • The contractor will conduct satisfaction surveys of family members and other representatives of long-stay nursing home residents by mail, with an online option for respondents. • The contractor will conduct resident quality of life surveys in assisted living settings through in-person interviews. • The contractor will conduct family satisfaction surveys in assisted living settings by mail, with an online option for respondents. • The contractor will conduct NCI-IDD and NCI-AD consumer surveys for individuals receiving home and community-based services via STATE-funded programs, waiver programs, Medicaid state plan, and Older Americans Act services.
