Description
This contract is for supportive services, including senior center activities and transportation, provided by Age Well Senior Services, Inc. to the County of Orange. The contract amount is $217,155, and the services will be provided from July 1, 2024, to June 30, 2025. The contract includes provisions for confidentiality and security of personal and sensitive information, as well as requirements for staffing, budgeting, and performance standards. Some key points include:
• The Subrecipient shall keep true and accurate accounts, records, financial and statistical data, which shall correctly reflect the business transacted by Subrecipient in accordance with generally accepted accounting principles.
• The Subrecipient shall protect CDA PCSI from unauthorized disclosure and shall not disclose, except as otherwise specifically permitted by the Contract and CDA Program Guide, any CDA PCSI to anyone other than CDA/OoA personnel or programs without prior written authorization from the CDA/OoA.
• The Subrecipient shall implement administrative, physical, and technical safeguards that reasonably and appropriately protect the confidentiality, integrity, and availability of CDA PCSI.
• The Subrecipient shall develop and maintain a written information privacy and security program that includes administrative, technical, and physical safeguards appropriate to the size and complexity of the Subrecipient’s operations and the nature and scope of its activities.
