Transportation, Mortuary and/or Cemetery Services

This contract is for transportation, mortuary, and/or cemetery services for decedents in Mohave County, Arizona. The contractor, Sutton Memorial Funeral Home, will provide these services as needed, including picking up human remains, transporting them to the mortuary, and performing final disposition arrangements. The contract includes a fee schedule, with rates for adult burial and cremation, as well as body bag and cremains plastic urn services. The total cost of the agreement shall not exceed $50,000.00. The contractor is also required to maintain certain insurance coverage, including commercial general liability, business automobile liability, and workers' compensation. The contract includes provisions for termination, renewal, and indemnification. • The contractor will provide transportation services for decedents, including pickup from the place of death or medical examiner's office and transport to the mortuary. • The contractor will perform mortuary services, including preparation of the decedent for final disposition. • The contractor will coordinate final disposition arrangements, including transportation to the cemetery and interment. • The contractor will maintain accurate records and provide reports as required by the county. • The contractor will comply with all applicable laws, regulations, and ordinances.
