Transportation, Mortuary and/or Cemetery Services

This contract is for the provision of transportation, mortuary, and/or cemetery services for indigent decedents in Mohave County, Arizona. The contractor, Lietz Fraze Crematory (LHC), will provide these services in accordance with the terms and conditions of the contract, which includes a fee schedule and requirements for insurance and subcontracting. The contract has a term of four years, with the option for renewal, and the total cost of the agreement shall not exceed $50,000.00. The key services provided under this contract include: * Transportation of decedents to the mortuary * Mortuary services, including preparation and cremation * Cemetery services, including interment and maintenance * Coordination with families and other providers to ensure respectful and professional services * Compliance with all applicable laws, regulations, and industry standards.
