Year Two, Older Americans Act Title III Area Plan Contract 2023

This contract is for the provision of National Family Caregiver Support Program Services to the elderly of Ocean County. The contract was awarded to HMH Hospitals Corporation - Southern Ocean Medical Center in an amount not to exceed $15,000.00. The contract term is from January 1, 2023 to December 31, 2023. The services provided under this contract include support for family caregivers of elderly individuals. • The contract is contingent upon the availability of funds. • The County reserves the right to cancel the contract at any time during the contract term if funds are not appropriated. • The contract number CC2023-28 must be placed on all papers, documents, invoices, and vouchers pertaining to the agreement. • The Director and Clerk of the Board are authorized to execute any necessary subsequent related documents.
