PRINT "APPLICATION & RENTAL AGREEMENT" FORMS

The County of Hawaii is seeking a vendor to print and deliver 400 "Application & Rental Agreement" forms for the Department of Parks & Recreation. The forms should be 3-part NCR with changes, black ink except for the number in red ink, and numbered from 6701 to 7100. The vendor should provide proof before printing and sample forms are available for viewing at the Purchasing Division. The contract was awarded to Hilo Bay Printing Co., Ltd. for a total price of $142.71. • The forms should be 8 1/2" x 11" in size. • The vendor should provide a design/set-up fee if not already included. • Shipping and tax should be included in the price if not already included. • The contract is for a one-time delivery of the forms. • The County of Hawaii is exempt from federal taxes and the City and County of Honolulu has a 0.5% General Excise Tax Surcharge.
