The Government Publishing Office is seeking a vendor to produce 60,000 confidentiality bookmarks for the National Health Interview Survey (NHIS). The bookmarks are flat‑sheet, head‑to‑head printed in four‑color process with gloss coating on both sides, trimmed to 2 ¾ in. × 8 ½ in., and packaged shrink‑wrapped in packs of 200. The order must include random “blue‑label” copies—one certified copy from each of 200 sub‑lots—packed separately with a blue‑label form. Finished products are to be palletized on wood pallets (max height 48 in., max carton weight 30 lb) and shipped to the National Processing Center in Jeffersonville, IN by August 18 2026. The contractor must submit an online quote, provide a PDF soft proof, ship two pre‑production samples for approval, and furnish required documentation (W‑9, Certificate of Selection).
