Western Oregon Service Unit Laboratory Supplies

The Indian Health Service is seeking a contractor to provide laboratory supplies on an as-needed basis to the Western Oregon Service Unit. The contractor will be required to deliver supplies within 10 business days of order, and the contract will have a base period of 12 months with four option periods of 12 months each. The contractor must provide all supplies listed in Attachment 1, and pricing must include all costs necessary to provide the requirement. The Government will evaluate quotations based on price and past performance, with price and past performance being approximately equal in importance. • The contract will be a firm-fixed-price commercial products contract. • The contractor must be an Indian Small Business Economic Enterprise. • The contractor must comply with all applicable laws and regulations, including those related to infection control, safety, and security. • The contractor must provide a completed IEE Form with the quotation. • The contractor must have a valid SAM.gov registration.
