Long-Term Care Ombudsman and Elder Abuse Prevention

The County of Stanislaus seeks a qualified organization to operate its Long‑Term Care Ombudsman Program and an Elder Abuse Prevention Program for the Area Agency on Aging. The contractor will provide advocacy, investigation, public education, training, data collection, and reporting services to protect residents of skilled‑nursing and residential‑care facilities, and to prevent elder abuse, neglect, and financial exploitation. The contract is funded jointly by Federal (HHS) and State Title III monies and will run for an initial one‑year term beginning July 1 2026, with the possibility of up‑to‑three‑year extensions. The award will be based on a technical qualification score (85 points) and price (15 points). • Provide 24/7 ombudsman access, investigate complaints, and submit data to state‑wide reporting systems. • Deliver elder‑abuse prevention outreach, training, and coordination with Adult Protective Services and law‑enforcement. • Manage a $261,537 budget, comply with federal, state, and County regulations, and maintain required insurance, certifications, and record‑keeping.
