Description

The Maryland Department of Health (MDH) is seeking a qualified, ADCES‑accredited or ADA‑recognized Diabetes Self‑Management Education and Support (DSMES) provider to improve referral pathways, increase enrollment, and enhance operational efficiency for DSMES services across the state. The contract is a firm‑fixed‑price, double‑envelope RFP covering a nine‑month period (Oct 1 2026 – Jun 29 2027) with up to two awardees. Work includes site assessments, development and implementation of standardized referral protocols, staff training, continuous quality improvement, and detailed reporting. • Conduct comprehensive site‑specific assessments of current DSMES referral, enrollment, and patient‑engagement workflows. • Develop and implement at least one standardized referral protocol aimed at a 10 % enrollment increase. • Provide training for providers and staff, including pre/post surveys to measure skill and knowledge gains. • Monitor key performance metrics (referrals, enrollments, conversion rates) and produce quarterly and final reports. • Deliver a project timeline, workflow improvement plan, baseline report, and final sustainability recommendations. • Attend kickoff and ongoing progress meetings with the CCDPC. • Comply with ADA accessibility requirements and all state procurement regulations.

Budget estimate

$500,000 – $1,500,000

The solicitation does not disclose a total contract value. Based on comparable state‑funded public‑health program improvement contracts, a 9‑month DSMES enrollment enhancement effort that includes site assessments, protocol development, training, data monitoring, and reporting typically ranges from $500,000 to $1,500,000. The lower end assumes a modest scope with limited subcontracting; the higher end reflects broader statewide implementation, multiple sites, and extensive data‑analytics support.
