In-Home Residential Services

The City of Norfolk is seeking one or more qualified contractors to provide In-Home Residential Services on an as-needed basis for individuals receiving services from the Norfolk Community Services Board. The services are designed to build skills in daily living activities and ensure the health and safety of individuals in their homes. The contractor will be required to participate in the development of an Individual Service Plan and provide a written "Plan for Support" for each client. The contractor will also be required to provide routine support, skill-building, and safety support, as well as monitor the individual's health conditions and provide transportation to and from community sites and resources. • Provide In-Home Residential Services to individuals with intellectual and developmental disabilities • Develop an Individual Service Plan and a written "Plan for Support" for each client • Provide routine support, skill-building, and safety support • Monitor the individual's health conditions and provide transportation to and from community sites and resources • Submit daily documentation and quarterly reports on client progress
