Description
The Department of Veterans Affairs is seeking a contractor to provide Onsite Mobile Lithotripsy Services for the James H. Quillen VA Medical Center. The contractor shall furnish all personnel to provide services necessary to perform Onsite Lithotripsy Services to eligible beneficiaries of the James H. Quillen Veterans Affairs Medical Center. The contractor’s Certified Medical Laser Safety Officer(s) shall cover the range of Lithotripsy services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized standards for the field.

• Providing Onsite Mobile Lithotripsy Services for the James H. Quillen VA Medical Center
• Furnishing all personnel to provide services necessary to perform Onsite Lithotripsy Services
• Meeting or exceeding currently recognized standards for the field
• Maintaining professional liability insurance issued by a responsible insurance carrier
• Providing three business references who have first-hand knowledge of the contractor's performance relative to the same type of services involved for this requirement
