The Iowa Department of Health and Human Services seeks a Lead Entity per each of its seven HHS districts to coordinate, plan, and administer the State’s Priority Public Health Core Services (communicable disease control, environmental health, chronic disease and injury prevention). The Lead Entity will conduct a Phase 1 foundational analysis, develop an interim district public‑health service system plan, and then, in Phase 2, implement the plan, maintain a provider network, collect and report data, and collaborate with county Boards of Health and other state programs. The effort spans a contract period from January 1 2027 to June 30 2032, with a fixed‑price Phase 1 budget (~$590 k) and a reimbursable Phase 2 budget (district‑specific, up to several million dollars).
• Conduct district‑wide health system assessment, stakeholder engagement, and interim planning (Phase 1).
• Oversee implementation of the District Public Health Service System Plan, including data collection, provider coordination, and quarterly reporting (Phase 2).
• Serve as the primary fiscal and administrative conduit for Core Service funding, ensuring no gaps in service delivery across the district.
• Maintain active liaison with county Boards of Health, local public‑health agencies, and Iowa HHS program partners.
