This Iowa HHS solicitation funds the implementation of evidence‑based interventions (EBIs) to boost colorectal cancer (CRC) screening rates in Iowa clinics. The awardee will work directly with Federally Qualified Health Centers, community health centers, or hospital‑affiliated clinics to redesign clinical workflows, integrate CRC screening reminders and provider assessment tools into electronic health records, and provide patient navigation services that address barriers such as transportation, language, and cost. The contractor must deliver a detailed work plan, conduct training, provide ongoing technical assistance, and submit quarterly and annual performance reports documenting screening rates, EBI uptake, and navigation outcomes. The contract begins in January 2027, runs for an initial eight‑month term with up to five one‑year extensions, and is funded from a total ceiling of $516,000 for all awarded sites.
• Implement three mandatory EBIs (patient reminders, provider reminders, provider assessment & feedback) plus one supportive activity (small media, provider or patient education).
• Provide dedicated CRC Screening Champion, Patient Navigator, and EHR Change Manager staff (distinct individuals) and maintain required staffing percentages.
• Collect, aggregate, and report baseline, quarterly, and annual CRC screening data, including kit distribution, return rates, and follow‑up colonoscopy completion.
• Conduct training, technical assistance, and workflow assessments; develop or update organizational CRC screening policies; and create sustainability plans for multi‑year operation.
