Description
The Department of Veterans Affairs is seeking a Discharge Care Plan software solution to provide a brand name or equal product for a period of 12 months with four 12-month option periods. The software will be used to ensure continuity, scalability, and sustained impact on discharge processes across the facility. The contract will include the right to make and use a reasonable number of copies of the software for testing, backup, archival, and disaster recovery purposes, as well as documentation for internal training, support, and deployment.
