Key requirements include: • Pre-qualification by the Commission or another Executive Office of Health and Human Services (EOHHS) agency • Submission of a proposal, including resumes, business references, and other supporting documents • Compliance with CORI (Criminal Offender Records Information) requirements • Maintenance of an e-mail account for communication and submission of electronic signatures • Submission of original ink signatures on certain forms after contract award The scope of work includes providing Transitional Assistance services to eligible individuals, including assessing needs, arranging for goods and services, and supporting the transition to community-based living.
