Child and Family Team Facilitator services

The San Bernardino County, through the Human Services Department for Children and Family Services, is seeking proposals from interested and qualified individuals, organizations, and firms to provide Child and Family Team Facilitator services under a cost reimbursement contract. The contract will be for a term of 3 years, with a total estimated cost of $8,400,000. The services will include providing Child and Family Team Facilitator services to class children, including initial CFTMs and ongoing CFTMs. The contractor will be required to administer the CANS assessment report and provide services in accordance with the RFP. Some key points to consider: • The contractor will be reimbursed for each completed CFTM referral as per the fee-for-service schedule. • The average length of time for a CFTM is 1 and ½ hours to 2 hours. • The fee per CFTM is $250, with no difference in rate based on the number of youths. • The contractor will be required to provide services in English, Spanish, and other threshold languages as needed.
