TB Prescription Services - Lake Havasu Smiths

This contract is for medication prestock services between Mohave County Department of Health and Smith's Pharmacy #188. The vendor will provide anti-tuberculosis pre-stocked prescription medications to the pharmacy at no charge, and the customer will be billed monthly for all authorized prescriptions at $6.00 per prescription. The contract has a term of 2 years, with the option to renew for up to 4 additional years. • The vendor will segregate the pre-stocked medications and ensure they are only dispensed to Mohave County Department of Health patients or their authorized agents at no charge. • The customer will maintain the supply of pre-stocked medications and remove expired medications from the pharmacy. • The contract includes provisions for payment, termination, and indemnification. • The vendor must comply with all federal, state, and local laws, orders, and regulations applicable to the terms and conditions of the contract.
