Description
The City of Avondale has entered into a Professional Services Agreement with Southwest Behavioral & Health Services, Inc. to provide mental health support services to City staff, partners, and residents seeking services out of the Arizona Complete Health Avondale Resource Center. The agreement is for a term of two years, with the option to renew for up to four successive one-year terms. The services include personnel, professional development, equipment, and operations. The total annual compensation for the services is not to exceed $100,000.00. The agreement outlines the scope of work, compensation, payments, safety plan, ownership of documents, and termination procedures.
• The services will be provided by a full-time equivalent (FTE) staff member with expertise in mental health support services.
• The FTE staff member will provide training and education to City staff and partners on mental health issues and crisis intervention.
• The agreement includes a detailed fee proposal outlining the costs for personnel, professional development, equipment, and operations.
• The City of Avondale and Southwest Behavioral & Health Services, Inc. will work together to establish a clear referral process for counseling services and ensure accessibility to mental health support services.
• The agreement is subject to termination for convenience, cause, or due to work stoppage, and outlines the procedures for termination and payment.
