Description
The Colorado Department of Health Care Policy and Financing is seeking a Local Contact Agency to provide In-Reach Counseling services to Medicaid members residing in skilled nursing facilities. The selected agency will be responsible for conducting Group In-Reach sessions and Individual In-Reach counseling to provide information about community-based services and support. The agency will also be required to develop a Regional Resource Document and provide quarterly Regional Transition Coordination Meetings. The contract is expected to start at the end of the start-up period and will last until June 30, 2027. The Department is looking for an agency with experience in educating adults with disabilities about their benefit options and working with Medicaid members and community organizations.
Some key requirements of the contract include:
* Conducting at least one Group In-Reach session at each Medicaid-certified nursing facility in the region per 6-month period
* Developing and submitting a Group In-Reach Plan to the Department for review and approval
* Providing a Regional Resource Document that includes information on housing, food, and caregiver support, as well as diagnosis-specific resources and transportation options
* Participating in quarterly Regional Transition Coordination Meetings to discuss transition coordination with community partners
* Ensuring that all work performed under the contract complies with applicable provisions of the Accessibility Standards for Individuals with a Disability
