Description
The Michigan Department of State is seeking proposals for the provision of Optec 1000DMV Vision Screeners for use in Secretary of State Branch Offices. The contract will have a term of 3 years, with options for renewal. The successful bidder will be required to provide timely deliveries, adhere to the scope of work, and meet certain service level agreements. The State reserves the right to reconsider or amend service level agreements for split awards.
• The vision screeners must be delivered within 45 calendar days of receipt of order.
• The contractor must ensure that items and quantities delivered are exactly the items, brands, and quantities on the Order Confirmation.
• The entire order will be received on the same day unless a partial delivery has been approved in advance by the Program Manager.
• The contractor must adhere to all aspects outlined in the Statement of Work, as agreed upon in the final version of this contract.
• The acceptable standard is 100% compliance.
• Credit of $100.00 may be assessed for the first occurrence of non-compliance in a given calendar year, and $1,000.00 may be assessed beginning with the second occurrence of non-compliance and on each occurrence thereafter in a given calendar year.
